The right rail, ramp or piece of equipment is often the difference between staying home and not. Our occupational therapists assess, prescribe and see the job through - funded through the NDIS or the aged care Assistive Technology and Home Modifications (AT-HM) Scheme.

What we assess and prescribe
- Minor home modifications - grab rails, handheld showers, step wedges, lever taps
- Major home modifications - bathroom conversions, ramps, stair lifts, doorway widening
- Assistive technology - mobility aids, transfer equipment, beds, pressure care and daily living aids
- Vehicle and community access equipment where funded
Every prescription starts with an in-home assessment. We measure, trial where possible, and write the funding documentation to the standard the funder expects.

How home modifications are funded
NDIS participants
Home modifications and assistive technology sit in your Capital budget. We complete the OT assessment and the AT/HM evidence forms the NDIA requires, and we liaise with builders on quotes and compliance. See our NDIS services and functional capacity assessments.
Support at Home (aged care)
The AT-HM Scheme funds equipment and modifications up to $15,000 where assessed as needed - separate from your quarterly budget. We complete the assessment and prescription your provider needs to release the funding. More on Support at Home.

FAQs
How much does the AT-HM Scheme cover?
Up to $15,000 for assistive technology and home modifications under Support at Home, where an assessment shows they are needed. Clinical assessments like ours are funded separately as clinical care.
Does the NDIS pay for home modifications?
Yes, through your Capital budget, with OT evidence. Minor modifications move quickly; major modifications need more documentation and quotes - we manage that process.
Do I need a doctor's referral?
No. Anyone can refer - participants, families, support coordinators or aged care providers.
Which areas do you cover?
Northern NSW and South East Queensland: Tweed Heads and the southern Gold Coast, Ballina, Lismore, Casino, Grafton, Yamba and Coffs Harbour.
