Baseline assessments and progress tracking. Reablement starts with a functional assessment that establishes a baseline. Progress is measured against that baseline at regular intervals. This creates a clear evidence trail showing whether therapy is making a difference.
Structured reporting. Good reablement providers deliver weekly and quarterly reports that document what was delivered, what progress was made, and what the plan is going forward. This reporting gives your facility ready-made evidence for AN-ACC, accreditation audits, and continuous improvement processes.
Proactive clinical care. Reablement is inherently proactive — it's about improving function, not just maintaining it. This aligns with the AN-ACC emphasis on quality and effort, not just activity.
